We are always excited to see you in our office and we will do everything possible to be able to accommodate ourselves to your schedule. Please call our main office number, and our office specialists will help you to make an appointment. They will ask you several questions to facilitate the scheduling procedure.
Patients will only be seen by appointment, except in the case of an emergency, which we will try to take care of as soon as possible. If you need to change or cancel your appointment, we ask that you contact us at least 24 hours before your appointment. However, do not hesitate to call us even on your appointment day. We further ask that you contact us at least 7 days before your surgery if you need to cancel or reschedule.

For patients who were referred to our office by another doctor, please make sure you can inform us about the doctor's name and address. If your insurance requires a referral from that doctor, we request it before your visit. Unless otherwise stated, we routinely send your doctor a written report of our findings and treatment suggestions.
We want to make sure we have everything we need to ensure that you receive the best service possible. Be sure to bring your current insurance card to your appointment, along with its reference number, if necessary. Please inform your referral doctor about your appointment, so that any relevant medical records can be sent to us before your appointment.
A full and complete medical history is needed to accurately assess you and suggest any further medical treatment. If you are a new patient, please complete the Patient Registration Form and Medical History Form. You will be sent a link via email to fill this out electronically ahead of time. However, printable PDF forms can be found on this website before your visit, and you can take them with you. We ask that you try to have these filled out and signed prior to your arrival at the office for your visit.
This will save you time at the office during your visit. Please make sure to read the Notice of Privacy Practices document before your visit. Please plan ahead and make sure to bring any relevant medical records, X-rays or laboratory work that we have not received prior to your visit to your appointment. Please also review the Patient Portal Instructions to set up your secure private and personal patient portal for exchange of protected health information.
When you arrive at our office, one of our office specialists will begin your registration process.
Please review your registration form and enter the information needed to establish your medical record and bill your insurance company.
Once your registration procedure is completed, one of our staff members will accompany you to an exam room and will ask relevant questions related to your visit.
Our doctors make every effort to always be a part of your scheduled appointment and be available at your scheduled appointment time. However, the doctor’s schedule is often fluid and could suddenly change if an emergency surgery or hospital visit is needed.
We ask you to be patient and understand that the doctors will give you the amount of time you need for questions and examination on your visit.
We require you to pay at the time of your scheduled service for all visits and office procedures, including deductibles and co-pays for insured patients.
We require a deposit of estimated patient liability, including deductible and coinsurance before your scheduled elective procedure or before the first visit after emergency surgery.
We accept checks, Mastercard, Visa, Discover and American Express.
We also kindly ask patients to avoid paying in cash, whenever possible.
We will help you file an insurance claim. Also, we participate in many healthcare plans. Your payment statements are sent out at least monthly and balances that are not paid within 30 days are considered for collections.
Some arrangements must be made if you cannot pay in full during that period.
